While the political underpinnings of could be better at feeling interconnected at times, some key decisions actually matter in terms of gameplay and the story narrative. However, in a sea of decisions, quests, and activities, it can be hard to know what actually matters and what doesn't. After all, it's more than possible to join all the different factions in , even though, from a narrative point of view, they don't get along. That being said, there are some key points in the game and a few decisions that will have a lasting impact on a save file.

Many of the decisions made throughout the journey in will reflect the ending before the protagonist enters Unity. This ending to the campaign slightly differs from past titles but is generally in line with other Bethesda games like. The ending sees decisions made throughout the playthrough reflecting on the future and prosperity of the world and NPCs encountered throughout the game. Throughout the character's space adventure, they'll encounter several companions that can join the crew, four of which can be romanced in . New relationship-centric dialogue is added to encounters, and sleeping with your romantic partner in for a nice little stat buff is possible.

However, those who wish to practice polyamory in, unfortunately, can't, as selecting a romance in the game means maintaining a monogamous relationship and cutting off tertiary romantic ties. Forming a bond with a companion does, however, also open a new questline to solidify the relationship through marriage, with each character having a different commitment quest that ties into their background and history.
During the main UC questline, the protagonist is eventually presented with a decision to out Vae Victis in «A Legacy Forged.» Up until this point, they've been working with him to hunt down the remaining members of Red Devils to devise a plan to eradicate the Terrormorph threat across the Settled Systems. However, he later reveals his dark plans and reasoning behind eradicating the Londinion colony and unleashing the threat of Terrormorphs on New Atlantis, equating to a false flag operation for a boosted ego and personal gain.
